WHAT IS TUMMY TUCK (ABDOMINOPLASTY)?

Abdominoplasty, also known as a "Tummy Tuck" or simply "TT", is the surgery of the abdomen to remove the excess skin and fat that may accumulate after pregnancy, obesity or age. With a full abdominoplasty, benefits are also tightening of the abdominal muscles that may have become stretched out or lax over time. Even a new belly button can be obtained as a result! This isn't a simple procedure, and you should definitely be aware of the risks and post-operative care and attendance that will be needed.

The purpose of the Abdominoplasty operation (Tummy Tuck) is two-fold. First, to flatten the abdomen by tightening
the muscles and tissues of the abdomen under the skin and secondarily to remove excess skin and fat that is on the
abdomen. The abdomen tends to take on a rounded appearance with aging and after childbirth. The muscles and tissues
between the muscles of the abdomen become stretched. No amount of sit-ups or leg raises can remove this shape.

Abdominoplasty is an aesthetic surgical procedure designed to remove excess skin and fat from the lower abdomen and to tighten the abdominal muscles.

Abdominoplasty is a type of cosmetic surgery procedure where the excess skin, particularly on the abdominal area,
is removed. Also known as tummy tuck, abdominoplasty can significantly flattens the abdomen, making it look normal and appear well proportioned.

Is abdominoplasty painful?
You will probably feel some pain and discomfort for the first few days after surgery. Dr. will prescribe pain medication to minimize this discomfort.

Is an abdominoplasty permanent?
Abdominoplasty effectively turns back the clock on the negative effects of aging on your physique. It does not, however, stop that clock. As you continue to age, it is possible that you will begin to develop some of the same problems again. However, the cumulative effects of the problems will be lessened, meaning that you will always have a trimmer midsection following an abdominoplasty than you would have if you hadn't had the tummy tuck procedure.

What is the difference between an abdominoplasty and a mini-abdominoplasty?
As opposed to a full tummy tuck , a mini-abdominoplasty employs a shorter supra pubic incision and no translocation of the umbilicus. A candidate for a mini-abdominoplasty has skin redundancy mostly below the umbilicus with muscle laxity confined to the lower abdomen. Liposuction may also be a performed. As a general rule, the greater the amount and distribution of excess skin, the longer the scar and necessity to translocate the umbilicus and perform a full abdominoplasty.

What are the risks with abdominoplasty?
Every surgical procedure has risks, which is why it is so important to choose a qualified surgeon to reduce the risks involved and so that if an emergency complication arises the surgeon is able to react and control the situation in the proper manner.
As with any surgery, there are risks with a tummy tuck. Though rare, tummy tuck surgery poses the risk of blood clots, infection and noticeably loose areas of skin following surgery. Poor healing could potentially lead to a second tummy tuck operation. To avoid further possible complications any plans of future weight loss or pregnancy should be discussed before the tummy tuck procedure. A competent and qualified surgeon such as Professor Dr. Ashok Govila will do everything to mitigate the risk of tummy tuck side effects.

Where is abdominoplasty performed?
Tummy tuck procedure is performed as an inpatient in a hospital. My Patients has an option to select a Hospital like Lifeline Hospital, Medcare Hospital and Al Garhoud Hospital and tell Professor Dr. Ashok Govila to perform surgery at the selected hospitals. Over all patient care, comfort and facility cost will be different at different hospitals and so will be the package deal. Cost is also dependent on your weight and height along with the severity of the issue and whether or not the Tummy Tuck is performed with or without liposuction.

Who is the best candidate for abdominoplasty?
Someone who is in good shape but cannot seem to lose excess tissue around the abdomen with diet or exercise is a good candidate. Remember, abdominoplasty is not a substitute for weight loss. What are the risks of bdominoplasty? As with any surgical procedure, there are some potentially serious risks. Please refer to our section on Abdominoplasty Risks for more information.

When is Abdominoplasty usually performed?
The Abdominoplasty procedure is usually performed when a person is discontent with his or her appearance in the abdomen region. Although there is no "normal" age or time in one's life, it is most often sought after a woman has given birth and does not plan on having any more children. The reason that not having additional children is an important factor is that, after the extra skin and muscle tissue is removed; the abdomen may not be able to accommodate another child for at least one year.

Many of my patients in UAE are women who are done having children and are discontent with their ability to lose weight or eliminate the sagging skin around their mid-section.

How is Abdominoplasty performed?
Abdominoplasty is performed most commonly under general anesthesia or light sleep IV sedation. Local anesthesia is then used to numb the abdomen. The operation is performed by making an incision in the lower abdomen just above the pubis. The incision may go from hip to hip. It is placed in the bikini line so that it is well hidden. The skin and fat is dissected off of the abdominal muscles all the way up to the bottom of the rib cage.
Abdominoplasty surgery is designed to remove and re-drape redundant lower and middle abdominal wall skin and fat. A horizontal incision is made above the pubic region, extending outward toward the hip bones. When possible, Professor Dr. Ashok Govila will keep the incision within swimsuit lines. The underlying muscles are tightened by pulling them together and stitching them in place. Excess skin from the lower abdomen is trimmed away. When a large amount of loose skin is present, a second incision will be made around the navel. What are the different techniques for Abdominoplasty?
The different techniques pertaining to Abdominoplasty consist of the different incision shapes and placement. Although most surgeons attempt to place the incisions in the most inconspicuous area, some surgeons may not, so please discuss this with your surgeon beforehand. Some surgeons go completely horizontal and straight across, some use a 'V' shape, some a 'W' shape, and still others follow along the bikini line. Your body's needs will determine the incision placement.

Where are the scars after abdominoplasty?
There is always a scar around the navel. There is almost always a low horizontal scar across the lower abdomen
just above the hairline. Sometimes there is also a short vertical scar above the pubic area. A person whose problem
is only laxity of the muscles, rather than skin excess, may be able to have endoscopic abdominoplasty, leaving only
the scar at the navel and a short scar above the pubic hairline.

Is abdominoplasty surgery painful?
Yes, moderately so, because of the tightening of the muscles that is almost always included. All the patients are given strong narcotic pills to take at home, and limit their activities accordingly.

How to Prepare for Surgery: To prepare for surgery both physically and mentally, consult Professor Dr. Ashok Govila
so you’ll know what to expect before the operation, the day of the surgery, and afterward:

Surgical consultation. You’ll meet with the Professor Dr. Ashok Govila to discuss your medical history and the procedure. If you have any questions, ask at this time so you understand what the procedure will entail and what to expect from the recovery period. A medical examination may also be performed at this visit, and preliminary tests may be ordered.

Pre-surgical visit. Professor Dr. Ashok Govila will go over the final surgery details with you and give you instructions to follow (fasting - nil through your mouth for 8 hours before surgery) the night before your surgery. Inform him the medications you are taking in case you need to stop taking any of them prior to surgery.

Day of surgery. You will meet with your medical team to go over your medical history and the procedure. The
anesthesiologist will explain how the anesthesia works and what effect it will have on you. Double-check that the
information on your hospital bracelet is accurate. And if you have prosthetics, wear dentures, or use a hearing aid,
be sure to inform your medical team. Inform the team of any drug allergies if you have, if you are diabetic ,if you have thyroid problems ,if you smoke ,if you have hypertension.

Discharge after surgery. You will receive instructions on how to take care of yourself, when to have a post-operative appointment, and what prescriptions you may need. You will be provided with medications to be taken for 3 - 5 days after the surgery by the hospital pharmacy. You will need to arrange to have a family member or friend take you home after surgery. Within first 24 hours after surgery one is not allowed to drive.

Prepare for Surgery: Improving Your Outcome

To best prepare for surgery, take these steps in the weeks and days leading up to it:

If you smoke, stop. It's best to stop smoking at least two weeks before your procedure, but even a few days ahead will help. Inpatient surgery usually requires general anesthesia, and if you stop smoking your lungs will adjust better to the anesthesia. Quitting will also help reduce your risk of infection.

Medications. Carefully follow all of your surgeon’s instructions regarding if and when you need to stop taking any of your regular medication prior to surgery. People who take blood thinners such as warfarin (sold under Coumadin and other brand names) will likely have to stop before surgery.

Control your blood pressure. Surgery will be less risky if your blood pressure is at a healthy level. This can be a
particular concern for overweight patients. “If you are able to lose weight before your surgery, it may help reduce your blood pressure and make the surgery easier,” professor says.

Take it easy. Relax and try not to overdo it in the days before your surgery. It’s hard not to be anxious, but if you
are well rested and in good shape, you will be better able to withstand the physical toll of surgery. Make sure to sleep well on the night before surgery even if it means taking a small dose 5 mg of tablet valium.

After Surgery: Guidelines
You will be encouraged by Professor Dr. Ashok Govila to get out of bed and move around as soon as you can, since being immobilized can be a cause of deep vein thrombosis.

Be sure you know when to resume taking medications that you discontinued prior to surgery, and confirm that they won’t cause a negative interaction with any painkillers you might have been prescribed.

Follow all of your doctor’s instructions about what you can or can’t do whether the advice is about having sex, driving a car, taking a bath, or walking up and down stairs. Understand that it may take some time before you can get back to all your normal activities. Continue to watch your diet and keep your weight and your blood pressure in check until you can return to your regular exercise routine.

Remember, the better you prepare for surgery, the easier the procedure and your recovery will be.

Abdominoplasty - After your surgery

For the first few days, your abdomen will probably be swollen and you are likely to feel some discomfort which can be controlled by simple medication like Panddol and tyrnol one gram daily. Depending on the extent of the surgery, you may be released the same day or remain in the hospital for 3-4 days.

Our local patients of Dubai and (Abu Dhabi ,Alain, Ajman ,Sharjah) usually recover at home. Professor Ashok performs body sculpt on patients from around the world and some of them prefer to recover in their hotels for few days before traveling to their home country. Some choose to bring a family member or friend. Some patients hire a nurse or caretaker until they are comfortable on their own. Our hospital have arrangements for doctor on call 24 hours a day to attend to you in your hotel if need be.

You will be advised to use pressure (compression garments) on the treatment area for 8 hours a day up to a maximum of 12 weeks. Professor Dr. Ashok Govila also treats the tummy tuck scars for a extended period of up to 6 months by custom fabricated medicated pressure garments of pliable medicated plates under the garments which is ever is preferred by the patient. Darker the skin color stronger is the indication for proper follow up and anagement of the scar . White skin makes good scars. Dark skin makes bad scars.

Some irregularities on the surface and nodularity are sometimes unavoidable and these patients are recommended to take LPG or some kind of massage therapy to smoothen them down. With the persistent use of pressure garments which are updated on monthly basis these irregularities settle down in time. Fibrosis (hardness of nodules) , at the level of fat is just a scar that finally will become soft and take away the surface irregularity.
